A corrective action report is a record of a problem, an investigation and a change, written so that the same problem does not return. In a board shop it is usually triggered by a customer complaint or by an internal nonconformance, and the quality of the response is decided in the first hour rather than in the paperwork that follows. Shops that treat the report as a form to be completed produce documents; shops that treat it as an investigation produce fewer repeats of the same defect.

Starting With Containment

The first action is always containment, and it comes before any investigation. Containment means finding out what else might be affected: other boards from the same batch, other batches from the same machine, and any material already shipped. It is an inventory question rather than a technical one, and it should be answered within hours because every day of delay increases the chance that more affected product reaches the customer.

Containment should also be recorded, because the record is what proves that the shop acted promptly and it is also the source of the sample that the investigation will use. Holding a known bad board and a known good board from the same batch is worth more than any amount of description, and it should be part of the containment step rather than an afterthought once the report is being written.

Describing the Nonconformance

A nonconformance should be described in terms of what was observed and what was required, with the measurement and the location. A statement that a hole wall is defective is an opinion, while a measurement showing insufficient copper on the barrel in three boards from one panel is a fact that the investigation can work with. The description has to survive being read by someone who was not there, months later.

The description should include the quantity affected, the process step where it was found, the step where it probably originated if that is known, and the quality criteria that were not met. Putting those together at the beginning makes it much easier to identify whether the issue is a single event or part of a pattern, and that distinction changes both the severity of the response and the effort that is justified. A single event on a prototype and the same event on a production lot of two thousand panels deserve very different amounts of work, and the report should make that judgement easy to reach.

Finding the Root Cause

Root cause is the condition that, if changed, prevents the recurrence. It is not the operator, and it is rarely the first explanation. The investigation should work through the process rather than the person: which parameters, which material, which machine and which instruction. A cause that names a person cannot be corrected, while a cause that names a parameter, a tool or a piece of documentation can be.

The useful discipline is to ask why until the answer is something the shop controls. Insufficient plating thickness on a barrel can be traced through the current density, the bath analysis, the panel loading and the hole condition, and one of those will be a condition that can be corrected and verified. Stopping the questioning early produces a fix that addresses the symptom and leaves the cause in place, which is how the same defect comes back under a slightly different description.

Writing the 8D Report

An 8D report is the structure most often requested by customers, and its strength is that it separates the immediate fix from the permanent one. The immediate action protects the customer now, the root cause explains why it happened, and the permanent action changes the process so that it does not happen again. Reports that present only the immediate action are the ones that generate a second complaint.

The report should be written in plain language, with the evidence attached rather than summarised away. Photographs, coupon measurements and machine records belong in the appendix, and the body should explain what was found and what was changed. A short report with clear evidence is accepted far more readily than a long one that repeats the customer’s own description back to them in formal phrasing.

Permanent Action and Verification

The permanent action has to be specific enough to be verified. Changing a work instruction, adding a check to the traveller, modifying a bath analysis frequency or adding a control to the machine program are all permanent actions, and each can be confirmed at a later date. An action described as more careful working cannot be verified, and it will have no effect.

Verification should include evidence from production rather than only a statement that the action was taken. A measurement series over the next few lots, a check of the plating specification on subsequent coupons, or a review of the next batches produced by the same line all show whether the change worked. The date of the verification belongs in the report, together with the person who performed it, so that the closure is a fact rather than a claim. Where the verification shows that the action did not work, the report should be reopened rather than closed with a note, because a closed report containing an ineffective action is the most misleading document in the quality file.

Learning From the Report

Corrective action reports are most valuable in aggregate. A shop that reads its last twenty reports together usually finds that two or three causes account for most of them, and those causes are the ones worth addressing at the process level rather than one complaint at a time. This is where quality management becomes an engineering activity instead of an administrative one.

The reports should also feed the technical documents. A cause that turns out to be a margin that was too tight should change the internal specification, and a cause that turns out to be an easy mistake should change the drawing note or the fabrication notes. A shop that only files its reports has learned nothing, while a shop that edits its standards after each one gets progressively more difficult to surprise.

Practical Rules

Contain first, describe in facts, and investigate the process rather than the person. Use a structure that separates the immediate fix from the permanent one, attach the evidence, and verify the permanent action with production data before closing.

Review the reports together every quarter, and edit the specifications, the drawings and the defect list when the pattern shows what to change. The measure of a corrective action report is not how quickly it closes but whether the same customer complaint appears in the file again next year with a different batch number.
